Pulmonary involvement in ulcerative colitis is very rare and may present as a variety of disorders. Pulmonary disease in patients with inflammatory bowel disease may be due to lung involvement in the disease itself or to drugs used for treatment, such as sulfasalazine or mesalamine. The use of mesalamine was initially thought to be free of adverse effects. Recently, however, pulmonary diseases related to the use of mesalamine have been reported. In this report, we describe a patient with ulcerative colitis and mesalamine induced interstitial lung disease.
